The job involves:
Working with older people and adults with physical disabilities
assessment of their needs
creating care plans
empower individuals and aim to help them become more independent
liasing with nurses and carers to achieve the best possible care
monitor progress and review care plans
You can:
work independently
manage your own case load
speak English fluently
take up employment within the next 3 months in the UK
